Now Hiring PRN Licensed Social WorkerThe Licensed Social Worker plays an important and trusted role in the lives of our residents and their families. As the Social Worker, you will be actively involved in the pre-admission and admissions processes. You will listen to and act compassionately with residents and their family members while providing individual and group help when needed.Responsibilities of Licensed Certified Social Worker:Actively involved in the pre-admission and admissions processesListens to and acts compassionately with residents and their family members while providing individual and group help when neededMaintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needsInitiating appropriate interventions for residents experiencing mental and behavioral health issuesIdentifying individual social and emotional needsDevelop and maintains individualized care plansParticipate in the planning of the resident’s admission, community re-entry, or transfer to another facilityParticipating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the residentWork with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stayWork with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the communityQualifications for Licensed Social Worker:Bachelor’s Degree in Social Work or a related fieldLICSW or LCSW licenseStrong ability to manage, multi-task and communicate effectivelyLong-term care experienceJoin us as Licensed Social Worker today!We want to hear from YOU!
