We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join a collaborative healthcare team committed to delivering high-quality patient-centered care in the comfort of patients’ homes.
Key Responsibilities
- Conduct admissions, skilled nursing visits, and ongoing patient assessments
- Develop and manage individualized care plans in collaboration with physicians and clinical teams
- Provide hands-on patient care, education, and disease management support
- Monitor patient progress and adjust care plans as needed
- Coordinate care with therapists, home health aides, and other healthcare professionals
- Maintain accurate and timely clinical documentation
- Support patients and families with compassionate, holistic care
Qualifications
- Associate or Bachelor’s degree in Nursing
- Active and unrestricted Registered Nurse (RN) license
- Current CPR certification
- Minimum one year of nursing experience preferred
- Strong background in Med Surg, ICU, ER, or acute care preferred
- Home Health experience is a plus
- Excellent communication, clinical assessment, and patient care skills
