The Wraparound Case Manager (WCM) will be responsible for the participant’s intake, assessment, case plan, group sessions, and evaluation. The WCM will receive referrals form the Harris County Juvenile Probation Department. The WCM will identify participant stage of change using motivational interviewing, risks and needs, develop a wraparound plan, monitor progress, and coordinate referrals specific to the needs of the participant. The WCM will meet with the participant weekly and maintain regular contact to ensure the progress of each participant and their success, while identifying any barriers that may arise. The participant will be part of the caseload of 25 per year, for 6 months and will be tracked and monitored for 6-12 months after completing the program. This position requires a high level of family engagement. The WCM will also facilitate group sessions two times a week using the Voices curriculum for 12 sessions each cycle. The WCM will also facilitate, together with the Program Coordinator, a monthly check-in session with the probation officers to ensure smooth implementation of the VOICES program. This position requires the WCM to be responsible for documenting program acidity, entering data and preparing monthly reports reflecting case notes and such activity, as indicated. The WCM will report directly to the Program Coordinator.
JOB DUTIES AND RESPONSIBILITIES
- Outreach and recruit from Harris County Juvenile Probation department and community- based agencies who work with girls involved in the juvenile justice system to participate in the program
- Conduct wraparound case management activities to monitor the process, which includes frequent contact with the client, intake, assessment, intervention, and monitoring.
- Develop, foster, and maintain ongoing relationship with probation officers, community-based organizations, schools, and mentors
- Recruitment of adult mentors for girls who are referred to the program
- Actively engage the youth and family in the program
- Identify and create opportunities for parent education and engagement.
- Facilitate group sessions using the Voices curriculum two times a week per site.
- Conduct check-in session with probation officers bi-monthly for effective implantation of Voices
- Meet with community organizations for referrals and evaluation activities
- Prepare and submit reports in a timely manner per submission dates
- Attend training and workshops as they become available
- Initiate and plan community-based events to advocate for girls involved in the juvenile justice system
- Assist in evaluation activities, including data collection and follow-up
- Develop, implement, and monitor strategies to achieve goals
- Obtain and develop educational material and participant informational material, to enhance program activities and training
- Identify problems and take initiative to plan appropriate corrective action
- Identify opportunities to and actively collaborate with community partners
- Perform other duties assigned by coordinator in a timely and efficient manner
Qualifications
The following requirements are desired for this position:
- Minimum bachelor’s degree in social work, psychology, behavioral science or related field of study preferred
- Minimum of one- year successful experience in case management
- Ability to work with youth at-risk populations, specifically girls involved in the juvenile justice system.
- Excellent organizational skills and strong verbal/written communication
- Ability to adapt to a dynamic, changing environment
- Ability to travel to various sites weekly
Salary : $45,000 - $48,000
