CLASS TITLE: Community Paramedic

 

CHARACTERISTICS OF CLASS: This is a full-time position with the primary responsibility of providing advanced, patient-centered healthcare outside the traditional emergency response environment. This role focuses on bridging gaps in primary care, chronic disease management, preventive services, and social support for underserved or high-risk populations. The Community Paramedic collaborates with healthcare providers, social service agencies, and community partners to reduce unnecessary 911 calls, improve patient outcomes, and enhance overall community health. This position is EMS only; no firefighting activities

 

 

EXAMPLES OF WORK:

  1. Will NOT participate in firefighting activities.
  2. Learns all departmental rules and regulations and abides by the same.
  3. Performs all requirements to maintain certification/licensure as a Paramedic/Community Paramedic.
  4. Responsible for patient care reports.
  5. Studies all prescribed training materials as well as the location of City Streets and County Roads.
  6. Operates as both a member of a team and independently.
  7. Conduct in-home assessments, including physical evaluations, medication reviews, and safety checks.
  8. Provide chronic disease management support (e.g., diabetes, COPD, CHF, hypertension).
  9. Deliver preventive services such as vaccinations, screenings, health coaching, and wellness checks.
  10. Follow evidence-based protocols to address non-emergent medical needs and prevent hospital readmissions.
  11. Utilize telehealth technologies to coordinate care with physicians, nurses, and specialists.
  12. Respond to low-acuity 911 calls when appropriate, providing alternatives to emergency transport.
  13. Develop individualized care plans addressing medical, behavioral, and social needs.
  14. Coordinate referrals to primary care, mental health providers, social workers, and community services.
  15. Track patient progress and document all encounters in accordance with HIPPA and agency policies.
  16. Participate in multi-disciplinary case reviews and community health initiatives.
  17. Identify community health trends and participate in outreach programs addressing homelessness, substance abuse disorders, mental health, and at-risk seniors.
  18. Provide education on disease prevention, medication adherence, injury prevention, and healthy lifestyle practices.
  19. Serve as a liaison between EMS and community partners to strengthen continuity of care.
  20. Assist in developing community paramedicine protocols, workflows, and best practices.
  21. Collect and analyze patient data to evaluate program outcomes and performance metrics.
  22. Participate in training new staff and supporting public health emergency response
 

 

MINIMUM REQUIREMENTS: TRAINING AND EXPERIENCE:

  • High School graduate or GED.
  • Must have a valid Kentucky Operator's License and a good driving record.
  • Must be in good standing with the KY Board of EMS.
  • Must have and maintain up-to-date KBEMS Paramedic
  • Completion of IBSC Community Paramedic Certification within one year.
  • Current CPR and ACLS certifications.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, documentation, and interpersonal skills.

 

  PREFERRED REQUIREMENTS :

  • Experience in community health, primary care, case management, or public health.
  • Prior involvement in mobile integrated healthcare (MIH) programs.

SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES: Must be in excellent physical condition with no impairment. The candidate may have corrected vision. Must have the ability to understand and follow difficult oral or written instructions. Must have the mental capacity to retain and utilize instructions offered in lecture, demonstration, and practical exercise training presentations. Extended periods of driving. Special knowledge in these core competencies: Patient-centered care, Cultural humility, collaboration and teamwork, problem-solving and adaptability, documentation accuracy, and trauma-informed communication.

 

JOB RESPONSIBILITIES RELATED TO PATIENT PRIVACY

 

  1. The employee is expected to protect the privacy of all patient information in accordance with the City of Winchester’s Fire/EMS privacy policies, procedures, and practices, as required by federal [and state] law, and in accordance with general principles of professionalism as a health care provider. Failure to comply with the City of Winchester’s Fire/EMS policies and procedures on patient privacy may result in disciplinary action up to and including termination of employment or membership, or association with the City of Winchester.
  2. The employee may access protected health information and other patient information only to the extent that is necessary to complete their job duties. The employee may only share such information with those who have a need-to-know specific patient information you have in your possession to complete their job responsibilities related to treatment, payment, or other City of Winchester, Fire/EMS operations.

 

  1. The employee is encouraged and expected to report, without the threat of retaliation, any concerns regarding the City of Winchester’s Fire/EMS policies and procedures on patient privacy and any observed practices in violation of that policy to the designated Privacy Officer.

 

  1. The employee is expected to actively participate in City of Winchester, Fire/EMS privacy training and is required to communicate privacy policy information to coworkers, students, patients, and others in accordance with City of Winchester’s Fire/EMS policy.
