Statement of Duties

Under the direct supervision of the Social Services Manager (HHS), the HHS Case Manager will provide complete and customized case management for residents in need of services to support their mental, social, and physical wellbeing.

The City of Somerville provides a client-centered, strengths-based, and trauma-informed approach to case management services. Case managers are expected to approach this work as a partnership, recognizing the impact that trauma may have on an individual's experiences, choices, and engagement with services. Case managers offer information, options, and support while encouraging clients to take the lead whenever possible and appropriate. The goal is to address needs while building clients' confidence, understanding, and skills in a safe, respectful, and empowering environment.

Essential Functions:

The essential functions or duties listed below are intended only as illustrations of the various type of work that may be performed. The omission of specific statements of duties does not exclude them from the position if the work is similar, related, or a logical assignment to the position.

  • Conduct assessment and intake of the client in need of services and create a plan to address said concerns
  • Develop relationships and work with community based organizations, and other governmental agencies to identify the most appropriate and available resources and services for clients
  • Develop relationships and work with staff from other City departments to coordinate services for clients
  • Work in collaboration with other HHS (specifically the Human Services team) to create coordination of care plans
  • Provide referrals to other community agencies regarding needs such as mental health, housing, homelessness and substance use
  • Maintain client databases to track efforts and outcomes and produce required reports
  • Maintain up-to-date information and knowledge regarding social service resources offered by city, state and non-profit agencies
  • Develops strong partnerships with community organizations/service providers
  • Conduct workshops for internal City staff and external partner agencies on subject matters related to case management and oversight
  • Assists clients with navigation of forms and system to access programs and services
  • Assist with creating a seamless referral system to internal and external programs and services
  • Attend local coalition meetings on topics related to social service supports
  • Develop social media content, web content and publicity materials in collaboration with the Communications Department of the City of Somerville. Translate materials as needed
  • Perform related work as directed by the Deputy Director of HHS, or their designee

Recommended Minimum Qualifications

Education and Experience: Bachelor’s Degree and three to five (3-5) years’ experience in case management services; or any equivalent combination of education, training and experience which provides the required knowledge, skills and abilities to perform the essential functions of the job.

Licensed Social Workers (LSW) are strongly encouraged to apply.

Special Requirements

  • CPR/First Aid Certification w/in three months of hire and continually maintained thereafter
  • Position requires ability to travel independently to multiple locations within City to complete work assignments

Knowledge, Abilities, and Skill

Knowledge: Working knowledge of programs and resources familiarity with health and social issues; working knowledge of State and Federal programs and services to meet the needs of the community; strong knowledge of Microsoft Office Suite.

Abilities: ability to interact appropriately and sensitively with residents; ability to interact well with staff, participants and caregivers; ability to speak effectively and to communicate with individuals and groups; ability to handle problems and emergencies effectively; ability to adapt and be flexible in response to quickly to changing situations; ability to operate a computer; ability to maintain confidential information; ability to maintain, manage, and organize records; ability to work independently and set priorities.

Skills: Excellent oral and written communication skills; working hardware and software computer skills; skill in working with individuals with mental and/or physical challenges; patience and understanding. Bi-lingual or multilingual ability in Spanish, Portuguese, and/or Haitian-Creole is preferred.

Work Environment

The work environment involves everyday discomforts typical of offices, with occasional exposure to outside elements. Noise or physical surroundings may be distracting, but conditions are generally not unpleasant. Employee may be required to work beyond normal business hours in response to attend evening meetings or complete work assignments.

Physical And Mental Requirements

Physical Skills

Few physical demands are required to perform the work. Work effort principally involves sitting to perform work tasks, with intermittent periods of stooping, walking, and standing. May also be some occasional lifting of objects such as office equipment and computer paper (up to 30 lbs.)

Motor Skills

Duties are largely mental rather than physical, but the job may occasionally require minimal motor skills for activities such as moving objects, operating a telephone, personal computer and/or most other office equipment including word processing, filing and sorting of papers.

Visual Skills

Visual demands require constantly reading documents for general understanding and analytical purposes.

Hours: Monday - Wednesday 8:30am - 4:30pm, Thursday 8:30am - 7:30pm, Friday 8:30am- 12:30pm

Salary: $65,000 - $67,600
