General Description

The City of Madison Community Development Authority is looking to hire Service Coordinators!

The City of Madison Community Development Authority (CDA)/Housing Operations Unit of the Department of Planning & Community & Economic Development is hiring a Service Coordinators to support residents living in public housing and help strengthen their stability, independence, and overall well-being. This role provides day-to-day support to residents by connecting them with community services, responding to crises, and building relationships that encourage engagement and trust. Service Coordinators work closely with residents, property management staff, local service providers, and HUD-funded programs to make sure people have access to the resources they need. Key responsibilities include assessing resident needs, making referrals, coordinating services, and advocating for residents as they navigate employment, education, health and wellness, aging in place, disability support, financial stability, and other community resources. The position plays an important role in helping residents maintain stable housing and improve their quality of life, while supporting the CDA’s mission to provide safe, inclusive, and supportive housing for all.

Examples of Duties and Responsibilities

Resident Engagement and Assessment

  • Conduct outreach to residents to explain available supportive services and encourage participation.
  • Conduct needs assessments for residents to identify barriers to housing stability, self-sufficiency, and supportive service needs.
  • Develop and maintain individualized service plans in collaboration with residents, focusing on resident-defined goals.
  • Maintain ongoing contact with participating residents to monitor progress and adjust referrals as needed.

Service Coordination and Referral

  • Coordinate access to various community-based services.
  • Advocate on behalf of residents to help navigate complex service systems.

Supportive Services and Crisis Intervention

  • Organize and facilitate workshops, group activities, or informational programming for residents. Participate in the development and implementation of special resident service programs (I.e. donation programs, tax preparation, wellness, etc.). Encourage resident participation in community and wellness activities that promote social connectedness.
  • Facilitate resident education regarding CDA policies, procedures, and lease requirements.
  • Liaise between residents and CDA, and external community partners to enhance community resources and coordinate services and support. Assist residents with navigating forms and applications for benefits and programs.
  • Manage resident crises and concerns through appropriate interventions, referrals, and limited, direct supportive services for urgent or complex needs. Facilitate, mediate and help to resolve interpersonal conflicts and complaints among residents.
  • Assist residents in finding alternative housing when their current unit is no longer suitable

Collaboration and Partnerships

  • Work collaboratively with property management, maintenance, other service coordinators and housing staff to support housing stability and early intervention. Serve as a resource to property management and other housing staff on supportive services and community resources.
  • Support resident councils or leadership activities, as applicable.
  • Establish and maintain strong working relationships with local agencies, service providers, partner agencies and community organizations. Coordinate on-site programs, workshops, and events in partnership with external agencies.
  • Participate in interdisciplinary meetings related to resident needs, lease compliance concerns, or eviction prevention efforts.
  • Seek out and secure other resources which might improve the quality of life of CDA residents and help them to retain their housing and avoid homelessness.

Program Administration and Compliance

  • Ensure services are delivered in compliance with service coordinator program requirements, grant agreements and agency requirements.
  • Maintain accurate and timely case notes, service plans, and reports in compliance with HUD and agency requirements.
  • Track and report program outcomes, participation, and referrals. Track performance measures related to resident participation and self-sufficiency. Prepare monthly reports summarizing activities, resident outcomes, and program needs.
  • Participate in staff meetings, training, and professional development activities as required.
  • Assist in preparing and managing program budgets and grant applications. Assist with grant monitoring, audits, and reporting as required.
  • Seek new funding opportunities to expand or enhance supportive services offerings.

Perform related work as assigned.

Minimum Qualifications

  • Two (2) years experience in applying the following:
    • Housing programs and regulations as they relate to elderly, disabled, and low income populations.
    • Social service programs and systems associated with the elderly, disabled, and low-income populations, including mental health services available in the community.
    • Computers and ability to use computer software applicable to the duties of the position.
OR
  • Three (3) years of experience in providing related services to low-income individuals and families, e.g.; independent living skills training, nursing home placement, providing consultation and assistance in the resolution of personal and family problems, mental illness, alcoholism and other drug abuse, finances, resident disputes and related crisis intervention
OR
  • Bachelor's degree or higher level in a field such as social work, sociology, or a related field may be substituted for the experience requirements.

Special Requirements

  • Possession of a valid driver's license and reliable transportation. Employees are required to travel between CDA sites and are required to use their personal vehicle and will be reimbursed for mileage in accordance with the handbook.
  • Ability to attend meetings and other events outside of normal working hours. Occasional evening or weekend hours are required for resident meetings, agency meetings, training, or community events.
  • Ability to successfully attain Professional Service Coordinator certification through the American Association of Service Coordinators within twelve (12) months of hire. Failure to obtain the certification within the required time period may result in forfeiture of the position, absent extenuating circumstances.
  • Ability to meet the minimum training requirements for HUD Service Coordinators (36 hours within 12 months of hiring and 12 hours of continuing education annually thereafter).
Physical Requirements:
Employees in this classification are required to physically access and meet with residents in their apartments, including the ability to go up and down stairs. Employees must also have the ability to lift up to 25 pounds.

Working Conditions:
Work is performed in apartments of various conditions including, but not limited to, units with active pest infestations, unsanitary conditions, hoarding conditions, and exposure to second-hand smoke.
