What are the responsibilities and job description for the Community Resource Specialist position at City of Lincoln/Lancaster County?
The Community Health Services (CHS) Division is seeking a Community Resource Specialist (CRS) to support individuals and families in accessing health coverage, public benefits, and essential community-based resources. This position provides advanced human service work including case management, assessment, planning, advocacy, follow-up, and coordination of services for individuals experiencing complex social and economic barriers.
This position serves clients across multiple settings including on-site clinics (STI, WIC, Dental), in partnership with home visiting staff, through established community agencies, and in the jail setting. The CRS assists individuals with Medicaid and other benefit applications, conducts Social Determinants of Health (SDOH) needs assessments, resolves coverage barriers, and coordinates referrals to address food insecurity, housing instability, transportation, and other unmet needs.
Work involves developing and maintaining strong internal and community partnerships, participating in outreach efforts, and maintaining accurate and confidential documentation and reporting. The ideal candidate demonstrates respectful client-centered service delivery, professionalism, and the ability to work effectively with justice-involved individuals and other individuals facing complex barriers using a trauma-informed, nonjudgmental approach. Strong organizational skills, accurate documentation practices, and the ability to manage multiple priorities in a fast-paced public health environment are essential.
Graduation from high school or equivalent supplemented by college-level coursework in human or behavioral sciences, or other specialized training with six months of experience in a capacity related to the position and program to which assigned; or any equivalent combination of training and experience that provides the desirable knowledge, abilities and skills.
