Community Resource Specialist
Human Services Department, Division of Youth
The City of Lakewood is currently accepting applications for the position of full-time Community Resource Specialist in the Department of Human Services, Division of Youth. This position serves as the resource and referral source for the Community Based Services Program. Hourly rate is $25.29.
Qualified applicants will possess the knowledge, experience and ability to perform the following essential job functions:
- Provides information, referral and short-term case management to families who need supportive services and/or resources such as clothing, housing, utilities, food and other material goods.
- Documents and tracks referral sources, numbers, types of referrals.
- Maintains files required by the Collaborative Lead Agency and Children and Family Services.
- Coordinates and schedules family team meetings and visits, insures a home-like setting for family visits.
- Provides support to program staff in preparing emergency assistance and home visits.
- Participates in all required training, and team decision meetings.
- Assists families going through the eviction process.
- Supports other grant funded staff in meeting all of the grant contract deliverables.
- Knows, gathers and assists in disseminating resource information to the community.
- Assists with the planning, coordination and implementation of programming/events including required documentation.
- Research for targeted resource information as needed.
- Attends local and county-wide meetings and events to gather information and make resource connections.
- Assists in developing and maintaining an updated community resource guide.
MINIMUM REQUIREMENTS
- High School Diploma (Bachelor’s Degree in Social Work or related field preferred);
- Must have experience working with a diverse cultural population;
- Knowledge of child welfare systems and other child-serving systems;
- Knowledge of community resources;
- Must possess good computer skills;
- Must have the ability to work autonomously and with a team approach;
- Knowledge of local community and Greater Cleveland community a plus; and
- Must successfully pass a BCI&I fingerprint background check, drug screening and FBI background check, if necessary.
THIS IS A GRANT FUNDED POSITION WHICH IS RENEWED ON AN ANNUAL BASIS. FAILURE TO RENEW WILL RESULT IN THE ELIMINATION OF THE POSITION.
GENERAL INFORMATION:
This position requires some flexibility of scheduling. The selected candidate will be required to work some evenings and weekends.
