Job Summary
Under the direction of the Board of Health, Director of Public Health, Outreach & Prevention Coordinator, and/or their designees, the Outreach Clinician/Case Manager will work as part of the Street Outreach Team to provide assessment, referral, and coordination services for unhoused individuals in Fitchburg experiencing substance use disorders and/or mental health challenges. This position will serve as an integral member of a progressive public health team—including the Health Director, Street Outreach Team, and Fitchburg Police Department’s Community Engagement Unit—providing outreach and services in a variety of settings. The Clinician will also assist the Fitchburg Police Department with follow-up outreach and visits to individuals with co-occurring disorders and will be based at the Fitchburg Health Department Street Outreach location.Full Job Description
