Job Description
Set appointments to assess and reassess older adults for the Home Delivered Meal program. Track all data in computer system as necessary for grant requirements. Refer clients and caregivers to other services and agencies as necessary. Work with staff to set up meal delivery. Communicate effectively with families, caregivers and older adults. Other duties as assigned.
