YouthLink - Resource and Assessment Center is a newly established nonprofit 501(c)(3) serving Adams County, CO, dedicated to strengthening families and supporting youth through early intervention, assessment, and coordinated access to community resources. YouthLink accepts referrals from law enforcement when concerning behavior is identified and provides timely assessment, trauma-informed care coordination, and connections to culturally responsive supports aligned with each young person’s strengths and family context. Serving youth ages 12–17, YouthLink delivers early, preventative support before challenges escalate into school suspension, court involvement, or emergency response.

Partnering with young people and their families to connect them with individualized, culturally responsive supports
DEFINITION
This position provides programmatic leadership and operational oversight and partnership representation for YouthLink, a nonprofit 501(c)(3) serving young people and their families in partnership with agencies and community stakeholders across Adams County. In collaboration with the Executive Director, drives organizational mission, vision, and strategy.
The Outreach Specialist works closely with the Intake Specialist, Case Manager and Program Director to advance YouthLink’s mission and priorities.
ESSENTIAL RESPONSIBILITIES AND DUTIES
1. Community Resource Navigation and Oversight
Connect youth and families to vetted community-based services, supports, and opportunities aligned with their needs and goals.
Monitor referral progress and ensure services are appropriate, accessible, and high quality.
Advocate for youth and families to reduce barriers to service access and engagement.
Maintain knowledge of community resources, service providers, and eligibility requirements.
Coordinate and support school engagement, including oversight of school coordination and summer programming support.
Follow up with providers and families to ensure successful service connection and continuity of care.
2. Cross-Agency Coordination and Documentation
Collaborate with partner agencies, committees, and service providers to coordinate care and support positive outcomes.
Participate in case coordination meetings, multidisciplinary teams, and community workgroups.
Maintain accurate, timely, and compliant case records, including service connections, contacts, and outcomes.
Track and report required program metrics to support accountability and continuous improvement.
Ensure documentation reflects trauma-informed and culturally responsive practices.
3. Action Planning, Safety, and Follow-Through
Co-create simple, goal-oriented action plans with youth and families, including connections to groups, counseling, school support, and employment or skill-building opportunities.
Provide mentoring, encouragement, and consistent engagement to support youth progress and motivation.
Support safety planning and apply harm reduction strategies as needed.
Assist with transportation or coordination of transportation resources to support participation in services and activities.
Maintain consistent follow-up to ensure plans are implemented and goals are progressing.
Identify challenges or risks and communicate concerns to the appropriate team members.
4. Program Support and Service Quality
Ensure outreach services are delivered in alignment with YouthLink’s mission, model, and quality standards.
Promote a trauma-informed, culturally responsive, and youth-centered approach in all interactions.
Identify service gaps, emerging needs, and barriers and escalate to the Program Director.
Participate in team meetings, training, and continuous improvement efforts.
QUALIFICATIONS
Knowledge of:
Trauma-informed care, culturally responsive practices, and positive youth development.
Community resources, youth programs, and service systems (Adams County preferred).
Resource navigation, outreach strategies, and engagement techniques.
Basic data tracking, documentation, and reporting practices.
Ability to:
Work nights and weekends as needed (anticipated schedule being Wednesday-Saturday or Sunday-Wednesday)
Build strong, trust-based relationships with youth and families from diverse backgrounds.
Engage youth in a supportive, strengths-based, and nonjudgmental manner.
Coordinate across multiple systems and service providers effectively.
Communicate clearly and professionally with partners, families, and team members.
Stay organized and manage multiple cases and follow-ups.
Demonstrate problem-solving skills, flexibility, and sound judgment.
Work independently while contributing to a collaborative team environment.
Maintain professionalism, confidentiality, and accountability.
EDUCATION AND EXPERIENCE
Bachelor’s degree in social work, human services, psychology, criminal justice, or a related field – Preferred.
2–4 years of experience in outreach, youth services, mentoring, case support, or community-based programs – Preferred.
Equivalent combination of education and relevant experience will be considered.
PHYSICAL REQUIREMENTS
While performing the duties of this position, the employee is regularly required to sit, stand, walk, talk, listen, and operate a computer. This role includes community-based work, travel to schools and partner sites, and may require lifting up to 10 pounds.
