The Case Manager (CM) coordinates care to individuals living with HIV/AIDS. The CM provides a range of client-centered services to link clients with health care, psychosocial, substance use services, and other community services. This position is part-time with an anticipated hours per week of 25 to 28 hours, that may change based on business needs. The pay range for this position is $24.45 to $32.90 per hour. Part-time employees accrue Earned Paid Leave time, and are eligible for benefits such as dental, vision, and life insurance; Employee Assistance Program (EAP) benefits, and wellness incentives.
