Position Summary:
The Case Manager for Domestic Violence Survivors will provide comprehensive support to individuals and families affected by domestic violence. This role is essential in helping survivors navigate their options, access resources, and achieve safety and stability. The Case Manager will work closely with clients to create personalized action plans, connect them with relevant services, and advocate for their needs in a trauma-informed and compassionate manner.
Key Responsibilities:
- Client Assessment: Conduct thorough assessments of clients to determine their immediate and long-term needs, risks, and goals related to domestic violence.
- Support and Counseling: Provide emotional support and crisis intervention to clients, offering a safe space for them to share their experiences and feelings.
- Resource Coordination: Connect clients with appropriate resources, including legal assistance, housing, healthcare, counseling services, and financial aid.
- Action Planning: Collaborate with clients to develop personalized safety and service plans that address their unique circumstances and empower them toward independence.
- Advocacy: Advocate on behalf of clients with service providers, legal systems, and community organizations to ensure they receive the support they need.
- Documentation and Reporting: Maintain accurate and confidential case records, documenting all interactions, progress, and outcomes. Prepare reports as required by funding sources or organizational policies.
- Collaboration: Work closely with a multidisciplinary team of professionals, including social workers, therapists, and law enforcement, to coordinate comprehensive care for clients.
- Crisis Management: Respond to crisis situations with urgency and sensitivity, providing immediate support and intervention as needed.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (Master’s degree preferred).
- Experience working with survivors of domestic violence or in a related social services field.
- Knowledge of domestic violence issues, trauma-informed care, and community resources.
- Strong interpersonal and communication skills, with the ability to empathize and build trust with clients.
- Ability to work independently and as part of a team in a fast-paced environment.
- Excellent organizational skills and attention to detail.
- Proficient in Microsoft Office Suite and case management software.
- Bilingual (Spanish/English) is a plus.
