What are the responsibilities and job description for the Case Manager (Careers Specialist) position at City Net?
Overview: The Case Manager provides first-level case management and supportive services to clients who are exiting street-level homelessness as residents of non-congregate interim housing. Case Managers (CMs) seek to connect guests and people experiencing homelessness to meaningful employment, career paths, and sustainable housing. CMs meet clients where they are and support them to achieve their goals.
City Net’s services are housing-focused. CMs also connect guests to appropriate supportive services, including medical treatment, mental health treatment, counseling, and permanent housing. CMs offer care and service coordination in partnership with Lead CMs, other nonprofits, County and City entities, law enforcement, behavioral health services, and medical providers to ensure clients receive the help they need. These services are provided in collaboration with our partners with the intention of assisting clients with obtaining and maintaining permanent housing as quickly as possible.
City Net’s programs follow Housing First and Harm Reduction service models.
Main Work Location: Reports daily to the Sacramento office to be dispatched to fieldwork throughout Sacramento.
Schedule: 5/8s (5 days each week, 8 hour shifts), Monday-Friday
This is not a remote position. The incumbent works full-time (40 hours/week), based out of our Sacramento office, with a specific weekly work schedule to be determined in consultation with their supervisor. This position requires daily driving in a company vehicle throughout Sacramento County.
Requirements
- Ensure qualified candidates are matched with roles that suit their skills and experience
- Providing career counseling, resume assistance, and job interview training
- Establishing relationships with employers within the community to maximize job and training opportunities
- Conducts job development and job search activities directed toward positions that are individualized to the interests and uniqueness of the people on his/her caseload.
- Provides individualized follow-along support to assist clients in maintaining employment; creates job support plans.
- Upbeat passion for ending homelessness
- Must be eligible to drive a company vehicle to transport clients. For insurance purposes, drivers must be at least 25 years of age, have a valid CA Drivers License, and must pass a Motor Vehicle Report

- Ability to work well with people from diverse backgrounds with varying degrees of experience
- This position may involve variable scheduling including early mornings, late evenings, and weekends. For example, there may be an early morning shift once per week, or a weekly evening outreach. Must be available for this type of variable schedule
- Must be able to pass a background check — at time of hire and throughout employment — in accordance with local laws/regulations
- Must be authorized to work in the United States for any employer
Nice To Have
- Experience interacting with people affected by homelessness, substance use disorders, chronic health, and mental health concerns, domestic and intimate partner violence, and/or human trafficking strongly preferred
- Spanish Fluency
- High School Diploma or Equivalent preferred
- Direct service experience in a shelter, group home, detox/recovery, healthcare, or other location serving vulnerable populations preferred
