Citizens Memorial Healthcare Foundation (CMH) is recruiting for a Social Service Designee to join the dynamic team of providers already in place providing exceptional patient care at the Parkview Healthcare Facility.
Parkview Health Care Facility (PVHCF) is a 78-bed long-term skilled nursing facility. A 15-bed memory care unit meets the specific needs of individuals with Alzheimer's disease and other forms of dementia.
A typical day will include: Provides Social Services to residents; You will stay with residents throughout their stay at the facility and will be responsible for assisting with completion of the admission process and working with the interdisciplinary team on discharge planning.Actively and consistently contributes to facility operations communications; behaves in a manner consistent with the mission and objective of CMH and performs other duties as requested.
The position is Full Time, Days

Education: Bachelor’s degree in Social Work or related field preferred, MHCA Designee Training required
