Citizens Memorial Hospital (CMH) in Bolivar, MO is hiring a Medication Management Clinic Pharmacist. The ideal candidate will be licensed in Missouri, have a Bachelor's Degree or PharmD and Medication Therapy Services Certificate. This role provides and oversees clinical pharmacy services by ensuring safe, effective, and accurate medication therapy for patients. The pharmacist will perform medication reconciliation, support optimal 340B utilization, and collaborate closely with providers across the organization. This position also helps maintain Joint Commission and ACHC quality standards, offers education and training to staff, and may supervise pharmacy technicians.
