Citizens Memorial Healthcare Foundation (CMH) in Bolivar, MO is hiring a Social Services Director to care for our residents at Ash Grove Healthcare Facility CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. About the facility: Ash Grove Healthcare Facility is an 82-bed long-term care, skilled nursing facility. Services include a 20-bed memory care unit to meet the needs of individuals with dementia, including Alzheimer's disease.
A typical day includes:
- Assisting residents and family with financial and legal matters
- Assisting residents to determine how they would like to make decisions about their health care, and whether or not they would like anyone else to be involved in those decisions
- Regularly attending care plan meetings and Medicare meetings
- Providing or arranging provisions of needed counseling services
Job Requirements:
- Education: Bachelor's Degree in Social Work or related field required in facilities with 120 beds or more
- Experience: LTC experience preferred
- Skills: Organization, Multi-Tasking
- Licensure/Certification: Social Services Designee
- Physical: Ability to stand for 60% of the day and sitting with residents for the remaining 40%
