Job Summary
The Registered Nurse (RN) is responsible for initiating, providing, and coordinating skilled nursing services in the home setting according to the physician’s plan of care, CMS Conditions of Participation, and agency policies. The RN performs comprehensive assessments, develops and updates the patient’s individualized plan of care, provides direct skilled interventions, supervises clinical staff, and ensures timely, accurate documentation to support safe and compliant care delivery.
Monday-Friday 8am-5pm
Weekend On-Call rotation
