Essential Duties
Duties include, but are not limited to:
Primary Care Responsibilities (Majority of Role)
• Provide high-quality, patient-centered primary care to adults experiencing homelessness, including diagnosing and managing acute and chronic medical conditions.
• Assess patient health through interviews, examinations, and review of medical histories.
• Order and interpret diagnostic tests, such as x-rays, EKGs, and laboratory studies.
• Develop and implement patient treatment plans, ensuring continuity of care and smooth care transitions.
• Instruct and counsel patients on therapeutic regimens, preventive care, and health maintenance.
• Collaborate and consult with supervising physicians and interdisciplinary team members as needed.
• Document patient care services in the electronic health record in a timely and accurate manner.
• Maintain compliance with infection control policies, safety standards, and regulatory requirements.
• Work collaboratively and respectfully as a core member of the interdisciplinary healthcare team.
Memory Clinic Responsibilities (One Day per Week with Potential to Expand)
• Serve as the medical provider for Circle the City’s Memory Clinic, conducting cognitive evaluations and ongoing management of dementia and related conditions.
• Work collaboratively with an integrated team consisting of behavioral health, social workers and community partners to address medical, mental health, and social needs.
• Provide trauma-informed, person-centered care to seniors with cognitive impairment, complex medical issues, and co-occurring mental health challenges.
• Perform cognitive assessments and obtain collateral histories when available.
• Identify patients who may qualify for ALTCS and coordinate referrals as appropriate.
• May include seeing patients in both clinic and shelter settings
Qualifications:Qualifications:

Position requires extended periods of sitting and standing;

Position requires regular bending and reaching, including transfer of patients;

Must be able to lift, carry and push 20 pounds on a regular basis;

Must be able to lift and carry up to 50 pounds on an occasional basis;

Able to adapt to rapid, unpredictable work flow;

Able to focus on work at hand with attention to detail;

Ability to work with difficult patients and those with mental illness.
Experience and Education:
-
Graduate of an accredited Physician Assistant or Nurse Practitioner program.

Maintain licensure with the State of Arizona to practice as a Physician Assistant or Nurse Practitioner

Maintain board certification to practice as a physician assistant or nurse practitioner
-
Certified in Basic Life Support
-
Minimum of three years of experience in either Family Medicine, Internal Medicine or Emergency Medicine

Understanding of substance and mental health issues
-
Computer proficiency inclusive of ability to use electronic health records
-
Excellent communication skills
-
Experience in working with underserved populations highly desirable
-
Experience in a community health center preferred
