Job Summary
The Health Center Case Manager is responsible for coordinating comprehensive care to address the medical, social, and emotional needs of patients. Serving diverse populations, which includes individuals experiencing homelessness, chronic illnesses, mental health issues and substance use challenge, the Case Manager supports solutions, anticipates challenges and takes initiative in overcoming obstacles to care. The case manager will collaborate with the Behavioral Health Consultant (BHC) and Associate Behavioral Health Consultant (ABHC) to support attainment of care plan goals. The case manager provides patient care in a manner consistent with the values of Circle the City.
Key Responsibilities:
- Directs Circle the City (CTC) patients through the Arizona Long Term Care System (ALTCS) program eligibility and enrollment process in collaboration with CTC providers and community partner agencies.
- Provides proactive support to patients working towards attaining care plan goals including connection to mental health and substance use services, housing resources, medical care and other unmet social determinants of health in a collaborative patient centered manner.
- Proactively and efficiently manages assigned case load.
- Reports patient care plan progress and provides feedback regarding updating care plan.
- Facilitates and maintains a positive working relationship with health center staff to ensure successful patient outcomes.
- Coordinates and fosters collaborative relationships with community partner agencies.
- Serves as an advocate for patients to reduce barriers to care and services.
- Completes documentation within agency timeframes in the EMR system.
- Adheres to Health Insurance Portability and Accountability Act (HIPAA) guidelines.
- Other duties as assigned.
