Position Overview:
The Licensed Social Worker is an essential member of the interdisciplinary hospice team, providing psychosocial support, counseling, and resource coordination for patients and their loved ones. This part-time role offers flexibility while making a meaningful impact in end-of-life care.

Your Role:
As a Licensed Social Worker, you will:
- Conduct psychosocial assessments and contribute to individualized care planning.
- Provide emotional support, counseling, and crisis intervention.
- Assist patients and families with advance care planning and decision-making.
- Connect families with community resources, including financial, placement, and supportive services.
- Collaborate with nurses, physicians, chaplains, and the hospice care team.
- Support bereavement and grief needs as appropriate.
- Participate in hospice admissions and ongoing patient visits.

Qualifications:
- Degree in Social Work from an accredited program (required).
- Current Social Work license in the State of Nevada
- Minimum of 2 years of experience, preferably in home health, hospice, or geriatric care.
- Strong knowledge of community resources and healthcare systems.
- Excellent communication and interpersonal skills.
- Valid Driver’s License with an acceptable driving record.
- Ability to travel within primarily Washoe County
- Experience with electronic medical records preferred.
