Job Summary:

The speech pathologist/audiologist CFY is a qualified professional who evaluates and provides speech therapy services to patients in the clinic as prescribed by the attending physician.

Responsibilities:

• Provide assessments and treatment activities with patients within the parameters of the regulations outlined by ASHA regarding the Clinical Fellowship Year (CFY) with the appropriate supervision of a licensed Speech-Language Pathologist 
• Schedule and plan activities to provides rehabilitative therapy for speech and language disorders 
• Records type of treatment and patient’s reaction to it on clinical/progress notes, which are written on the day of the visit and incorporated in the patient’s record weekly 
• Maintains adequate records on all patient’s, including summary reports 
• Utilizes community resources and Agency personnel for proper referral 
• Selects and administers diagnostic and therapeutic techniques and materials 
• Instructs and counsels other health team personnel and family members in methods of assisting patient in improving, correcting, and accepting his/her disabilities 
• Attends case conferences as requested 
• Participates in staff development activities and in-service education as requested 
• Confirms, on a weekly basis, the scheduling of visits with the Patient Care Coordinator to coordinate necessary visits with other personnel 
• Notifies Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will all affect agreed services with the Agency 
• Participates with staff, patient, and physician in discharge planning activities and completes the speech therapy discharge summary within the designated framework of the Agency policies 

Requirements

Qualifications:

• Master’s degree in Speech-Language Pathology from an accredited program. 
• Certified member of the American Speech-Language-Hearing Association (ASHA) or eligible for CFY. 
• Current Texas license to practice as a Speech-Language Pathologist (SLP), or eligibility to obtain licensure prior to start date. 

Organizational Relationship:

The Speech Pathologist/Audiologist CFY reports directly to the Supervising Speech Pathologist

Special Requirements:

• Reliable transportation and ability to travel to patient homes within the assigned service area. 
• Valid Texas driver’s license and current automobile insurance in accordance with state requirements. 
• Ability to meet documentation and compliance standards as outlined by state and federal home health regulations.
