As a Nurse Practitioner – In-Home Primary Care, you will play a critical role in managing the health of patients with complex and chronic conditions, ensuring continuity of care, and building long-term relationships with patients and their families.
This role is ideal for NPs who are passionate about patient-centered care, value autonomy in clinical decision-making, and want to practice at the top of their license within New Jersey’s scope of practice and collaborative agreements.
- Provide comprehensive in-home medical care to patients, including health assessments, diagnosis, treatment planning, and medication management in compliance with New Jersey NP scope of practice and collaborative physician agreements.
- Develop and implement personalized care plans focused on chronic disease management, preventive care, and improved quality of life.
- Manage chronic conditions, medication regimens, patient education, and follow-up care to ensure continuity and positive clinical outcomes.
- Conduct patient and caregiver education on disease management, medications, lifestyle modifications, and care coordination.
- Build strong, trust-based relationships with patients and families by delivering compassionate, culturally sensitive care in the home setting.
- Travel within assigned New Jersey regions (Edison to Princeton and surrounding areas) to deliver in-home healthcare services.
- Collaborate with physicians, care coordinators, and interdisciplinary teams to ensure integrated, high-quality care.
- Maintain accurate and timely clinical documentation in the electronic medical record (EMR) system.
- Adhere to all clinical protocols, compliance standards, and regulatory requirements.
- Active and unrestricted New Jersey Nurse Practitioner license
- National NP certification (ANCC or AANP)
- Valid collaborative practice agreement (or willingness to establish one)
- Minimum 1–3 years of clinical experience in primary care, geriatrics, home health, or internal medicine (preferred)
- Strong clinical assessment, diagnostic, and patient communication skills
- Comfortable working independently in a home-based care environment
- Valid driver’s license and willingness to travel locally
- EMR proficiency preferred
