Overview
Be a part of an innovative community program! Join Circare’s Youth Assertive Community Treatment (ACT) Team, serving families and youth in Onondaga County. This multi-disciplinary team will work with children ages 10 to 21 and their families to provide intensive intervention to support complex needs, as well as keeping children at risk of hospitalization or residential care in their homes.
Circare is seeking a Youth ACT Team Leader, who will work in conjunction with the Youth ACT Psychiatric Nurse Practitioner, to be the cornerstone of Circare’s Youth ACT Team, managing and supervising the 8-person team, as well as enhancing the program processes and procedures. This community-based program position is full-time, during regular business hours, M-F, with periodic after-hours and weekend on-call duties. Circare is looking for a candidate who is committed to supporting children with complex mental health needs and their families and is experienced and comfortable with providing services in the home and in the community.
Eligible for a Sign on Bonus!
Pay Range: $65,000 to $75,000 year
Responsibilities
- Directly supervise Youth ACT Team staff
- Provide clinical direction to Youth ACT Team staff on individual cases
- Conduct regular individual supervision meetings
- Lead daily service planning meetings
- Direct day-to-day operation of the Youth ACT program
- Carry a small caseload, working directly with Youth ACT enrollees
- Ensures compliance and quaility
- Collaborate with medical staff on individual cases
- Collaborate with community partners
- Interviews/hires/trains new staff
Qualifications
- A licensed professional LCSW, LMSW, LMHC or LMFT with a master’s degree or higher in social work, psychology, rehabilitation counseling or a related field
- Will consider a licensed Registered Nurse with appropriate experience
- Prefer experience with multi-disciplinary in-home and community-based services
- Strong background in recovery-based and person-centered behavioral health services
- Excellent verbal and written communication skills
- Proficiency in Excel and other Microsoft products
- Experience using electronic health record programs
