Certified Peer Recovery Supporter
Reports to: Practice Manager
Position Summary
NeighborHub Health is seeking a compassionate and dependable Certified Peer Recovery Supporter to assist individuals recovering from substance use disorders, with a particular focus on people who are unhoused or at risk of becoming homeless.
The Certified Peer Recovery Supporter will use their personal lived experience of recovery to build authentic peer-to-peer relationships and provide hope, encouragement, mentorship, advocacy, and practical support. This position helps individuals develop recovery goals, connect with community resources, overcome barriers, and maintain long-term recovery.
The successful candidate must be comfortable working as part of an interdisciplinary healthcare team and providing services in a variety of community and clinical environments.
This position will divide its time equally between the:
- Street Medicine Team – 50%
- Medical Clinic – 50%
Essential Responsibilities
- Engage individuals in recovery across multiple clinical and community settings, including medical clinics, street outreach locations, homeless encampments, shelters, and mobile clinics.
- Build supportive, respectful, and authentic peer-to-peer relationships with individuals experiencing substance use disorders.
- Use appropriate personal recovery experiences to demonstrate that recovery is possible.
- Provide recovery education throughout all phases of the recovery journey, including:
- Pre-recovery engagement
- Recovery initiation
- Recovery stabilization
- Sustained recovery maintenance
- Assist individuals in identifying their personal interests, recovery goals, strengths, needs, and potential barriers.
- Support individuals in developing and advancing their own individualized recovery plans.
- Promote self-advocacy by helping individuals communicate their needs, goals, preferences, and objectives.
- Provide ongoing hope, motivation, mentorship, and encouragement.
- Identify internal and external barriers that may prevent individuals from fully participating in treatment, recovery services, or community resources.
- Help individuals develop practical strategies for overcoming barriers to recovery.
- Connect individuals with community resources that support their recovery goals, including:
- Recovery communities
- Mutual self-help groups
- Mental health and substance use treatment services
- Medical and behavioral health services
- Housing and social-service programs
- Educational and vocational resources
- Employment opportunities
- Cultural and spiritual resources
- Recreational and community-based activities
- Link individuals to appropriate professional services and community-based recovery supports.
- Encourage participation in meaningful community activities, including employment, education, relationships, physical activity, hobbies, and other self-directed interests.
- Maintain appropriate contact with individuals following discharge or completion of services to support continued recovery and re-engagement when needed.
- Collaborate with clinical staff, outreach staff, case managers, providers, and other members of the care team.
- Maintain accurate project logs, reports, records, and documentation in the appropriate electronic health record, files, and databases.
- Assist with transporting patients to medical appointments, job sites, social activities, or community resources when assigned.
- Protect patient confidentiality and maintain appropriate professional and peer-support boundaries.
- Perform other duties as assigned.
Minimum Qualifications
- High school diploma or GED.
- Three professional or personal letters of recommendation.
- Personal lived experience with a mental health condition, substance use disorder, or co-occurring mental health and substance use disorder.
- Current certification as a Certified Peer Recovery Supporter, or documentation that all applicable certification requirements have been completed.
- At least 12 months of successful paid or volunteer work experience completed within the past three years.
- Understanding of and respect for each individual’s unique recovery journey.
- Working knowledge of substance use disorder treatment, behavioral health services, peer-support practices, and the recovery community.
- Demonstrated commitment to supporting individuals in recovery.
- Ability to share personal recovery experiences appropriately and professionally.
- Ability to develop trusting relationships while maintaining appropriate boundaries.
- Ability to work effectively as part of an interdisciplinary healthcare and outreach team.
- Ability to work with people from diverse backgrounds and individuals experiencing homelessness, behavioral health conditions, substance use disorders, and other complex needs.
- Strong communication, organization, documentation, and interpersonal skills.
- Ability to work in clinics, shelters, encampments, mobile medical settings, and other community locations.
- Valid driver’s license and the ability to travel to community locations and transport patients when required.
Preferred Qualities
- Experience working with individuals experiencing homelessness or housing instability.
- Experience providing street outreach, peer support, recovery services, case management support, or community-based services.
- Familiarity with local recovery, housing, employment, transportation, behavioral health, and social-service resources.
- Experience documenting services in an electronic health record.
Work Environment
This position includes work in both traditional medical settings and community-based environments. The Certified Peer Recovery Supporter must be comfortable participating in street outreach, visiting shelters and encampments, working in mobile clinics, and interacting with individuals who may be experiencing significant personal, medical, behavioral health, or housing-related challenges.
