ABOUT NEIGHBORHUB HEALTH: NeighborHub Health is a Cincinnati-based Federally Qualified Health Center providing integrated medical, dental, behavioral health, and support services for people experiencing homelessness and individuals living with HIV. We deliver trauma-informed, culturally responsive care through a collaborative, whole-person approach.

POSITION SUMMARY: The Integrated Behavioral Health Specialist provides psychosocial support, behavioral health care coordination, crisis support, outreach, and community resource linkage. This role works closely with medical providers, therapists, prescribers, peers, and community partners to reduce barriers, strengthen treatment engagement, and support continuity of care.

WHAT YOU WILL DO - Complete psychosocial screenings, resource assessments, and care plans within assigned scope. - Respond to warm handoffs and same-day behavioral health needs with brief psychoeducation and supportive interventions appropriate to your credential. - Screen for suicide, self-harm, and acute psychiatric risk using approved tools; coordinate safety planning and crisis referrals with licensed clinicians. - Coordinate behavioral health referrals, follow-up, treatment engagement, and re-engagement after missed appointments. - Connect patients with housing, food, transportation, Medicaid, disability, legal, employment, and other community resources. - Support HIV/HCV linkage to treatment and community programs and track required outcomes. - Participate in street medicine and outreach activities in shelters, encampments, and other community settings. - Co-facilitate approved groups with an independently licensed clinician. - Maintain timely EHR documentation and participate in team huddles, case conferences, quality improvement, and grant reporting.

REQUIRED QUALIFICATIONS - CSWE-accredited BSW or higher social work degree. - Current Ohio LSW, or eligibility and willingness to obtain licensure within the timeframe stated in the offer letter. Licensed activities may not be performed until the required credential is active. - Knowledge of homelessness, trauma, poverty, substance use, co-occurring conditions, social determinants of health, and community resources. - Strong communication, organization, documentation, and care coordination skills. - Ability to work effectively in a fast-paced, integrated, trauma-informed environment. - EHR proficiency. - Valid driver's license and ability to travel to outreach locations. - BLS/CPR certification or ability to obtain within 90 days of hire.

PREFERRED QUALIFICATIONS - One to two years of behavioral health, healthcare, or social service experience, especially with people experiencing homelessness. - Experience in an FQHC, safety-net setting, street medicine, HMIS, coordinated entry, Housing First, Ryan White programs, or HIV/HCV services. - Training in Motivational Interviewing, trauma-informed care, harm reduction, or Mental Health First Aid. - Spanish/English bilingual skills.

WORK ENVIRONMENT: This position combines clinic-based work with regular community outreach. Outreach may involve shelters, encampments, outdoor environments, changing weather, unsanitary conditions, and potentially volatile situations. Occasional early-morning or evening scheduling may be required. The position requires the ability to sit, stand, walk, travel to outreach sites, and lift up to 25 pounds, with or without reasonable accommodation.

SCOPE AND SUPERVISION: Duties are assigned according to the employee's credential, documented competency, and organizational policy. A BSW who is not licensed performs non-licensed support and coordination functions only. LSW clinical services are provided within Ohio law and required supervision. This is not an independent clinical position.
