Responsibilities:
Provide direct patient care to residents, including medication administration, wound care, and vital sign monitoring
Develop and implement individualized care plans to ensure the unique needs of each resident are met
Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication
Participate in the development and implementation of policies and procedures to maintain high-quality care standards
Maintain accurate and timely records of patient care and medication administration
Identify and report any changes or concerns to the Director of Nursing or Administrator
