The Social Services Designee/ Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident/guest discharge planning.

Responsibilities may include:
Assist the facility with:
- Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.
- Attend the 72 hour care plan meeting
- Ordering equipment for the community setting
- Arranging home health nursing for after discharge, as needed
- Coordinating the medications for community use
- Coordinate the entire discharge, with IDT involvement
- Attending care conferences for sub-acute residents/guests
- Attending Medicare Meetings
- Assist in procuring any item related to the discharge process for residents/guests anticipated to be returning to the community.
- Initiate and coordinate completion of the Post Discharge Plan of Care
- Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.
Education and/or Experience:
- High school diploma/GED
- None
