The Resident Care Coordinator (RCC) ensures that Direct Care Staff are well trained and are continually performing at optimum levels with primary focus on supervising the medication delivery system. Works in tandem with the Health Services Coordinator and the Administrator to ensure continuity of care and clear/timely communication to care team. The RCC is to set the example for appearance, demeanor, work ethic and communication with co-workers, residents, families, and outside parties.

Essential Duties and Responsibilities:

1. Maintains confidentiality of resident personal medical information, respects resident's rights, privacy, and their right to self-determination. Abides by the established Policies and Procedures of Churchill as well as all regulations that govern our community.

2. Responsible for completing the monthly schedule for all Caregivers and Medication Technicians and ensuring that the total hours daily meet both Churchill's and regulatory standards. Works with Administrator to secure coverage for Call-Offs, Vacation and Open Shifts. In partnership with the Administrator, will personally cover open shifts as needed.

3. Report any noted changes in resident's condition/behaviors or normal routine to the Health Services Coordinator and the Administrator. Arranges and/or coordinates clinical assistance for residents attending scheduled appointments, procedures, tests, etc. This could mean contacting family/responsible party to provide escort or transportation, scheduling/re-scheduling appointments for residents, scheduling transportation, ordering DME, etc.

4. Schedule and send out notices for Care Conferences to residents and involved parties. Work with Health Services Coordinator in updating Care Plans as needed. In cases when either the Administrator or Health Services Coordinator are unavailable to attend a scheduled Care Conference, RCC will periodically step in to provide updates on services. Maintain ancillary schedules such as laundry and shower schedules for staff to follow and update as necessary.

5. RCC will be the primary trainer for all new hires for Caregivers. The RCC will partner with the Health Services Coordinator for training new Medication Technicians. RCC will continually review new hires with both the Administrator and Health Services Coordinator and discuss training needs and recommendations.

6. Routinely performs or assigns audits in the Medication Room for expired medications, incomplete processing of orders, sanitation of the medication cart, temperature logs for the refrigerators, and ensures the Medication Room is kept clean, neat, and professional always.

7. Coordinates with Medication Technicians that all daily, weekly, and monthly vitals for residents are completed, documented, and reported in a timely manner.

8. Ensures the service of meals in the dining rooms is staffed timely by caregivers for all meals and that caregivers abide by the rules and procedures set forth by the Dining Services Director.

9. RCC works with the Administrator in tracking staff attendance and reporting to the Business Office Manager in a timely manner. Will provide feedback on staff behavior and work performance for the purpose of Corrective Actions and/or Annual Performance Reviews. May be requested by the Administrator or the Health Services Coordinator to sit in on individual meetings with staff members pertaining to complaint investigations or Corrective Actions.

Qualifications/Experience:

  • Minimum of a High School diploma or GED. Prefer associate degree or related Certification.
  • Minimum 4 years' experience providing care to seniors with a minimum of 2-years as a Medication Technician.
  • Current First Aid certification and must maintain through employment.

Job Type: Full-time

Pay: $21.00 - $23.00 per hour

Expected hours: No less than 40 per week
