Key Responsibilities:
- Conduct initial and ongoing client assessments to develop individualized care plans.
- Monitor care delivery for quality, safety, and compliance with regulations.
- Communicate regularly with clients, families, and healthcare providers.
- Maintain accurate documentation and records.
- Current RN license
- Minimum 2 years of nursing experience (home care or supervisory experience preferred)
- Strong leadership, communication, and organizational skills.
- Compassionate, patient-centered approach to care.
