Weekend Wellness Nurse

Job Summary
We are seeking a dynamic and compassionate Weekend Wellness Nurse to join our dedicated assisted living community team. This vital role focuses on providing high-quality nursing care during weekend shifts, ensuring the well-being and safety of our elderly residents. The Weekend Wellness Nurse will manage a team of personal caregivers, ensure the safety and well being of the residents, communicate with families and support the Resident Service Manager with service plans, scheduling and delivery of service. This position is for Saturday and Sunday coverage, from 9am-5pm including two weekdays.

Responsibilities

• Monitor patients’ conditions closely, managing acute pain and responding promptly to urgent needs or changes in health status.
• Collaborate with interdisciplinary teams to develop and update individualized care plans for residents.
• Provide triage services by evaluating symptoms, prioritizing care needs, and coordinating with physicians or emergency services when necessary.
• Maintain detailed medical documentation in compliance with HIPAA regulations and facility standards as well as AGE requirements.
• Support residents with activities of daily living and oversee Self Administered Medication Management administration.

Experience

• Prior nursing experience in long-term care facilities, assisted living communities, or nursing homes demonstrates your ability to manage geriatric and memory care populations effectively.
• Familiarity with assisted living regulations, PACE programs a plus.

Pay: $30.00 - $35.00 per hour
