POSITION SUMMARY:

Christopher Community, Inc. seeks a Resident Service Coordinator to join its growing organization! The physical work location for this position will be Villa Scalabrini Apartments located in Syracuse, NY. 

CCI is a not-for-profit development and management company that affordable housing to families and seniors across Central New York. The Resident Service Coordinator (SC) works in conjunction with building staff to facilitate services that empower residents to age in place and remain as independent and self-reliant as possible. They may provide limited or general case management; information and referral; plan educational programs, and coordinate volunteer opportunities. The SC reaches out to all residents on a regular basis to assess needs, promotes involvement in community activities, educates residents on available services, and develops a network of contacts with service providers and agencies for resident referrals. The SC also monitors the provision of services to residents, providing documentation of their involvement, as well as community engagement efforts.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

· Regular outreach, assessment, and service management to building residents focused on community services, benefits, health and wellness. 

· Coordinates regular formal or informal educational opportunities for residents, families, and staff on available community resources, services, and appropriate aging-related topics

· Acts as a liaison between community agencies, service providers, and residents – encouraging residents in building formal and informal support networks.

· Works as a team member with the property manager and other housing management staff in serving residents/clients, and supporting their ability to meet their leasing obligations (while maintaining job description boundaries)

· Encourages residents to be proactive in meeting their social, psychological, and physical needs.

· Facilitates/uses the least restrictive intervention necessary to alleviate a problem situation and avoids creation of unhealthy dependence.

· May work with residents to monitor the delivery of their services to ensure they are appropriate, timely, and satisfactory.

· Provides general or limited case management for a resident when such service is not provided by the larger service community.

· Reports all suspected abuse situations to the appropriate agency.

· Advocates and may negotiate on behalf of residents for adequate, timely, and cost-effective provision of services – meeting with resident and service provider as appropriate.

· Assembles a directory of community services and makes it available to residents, families, and management.

· (Note) The SC is not to provide continuous support services directly (direct service) or assist with other administrative work normally associated with the property’s operations. Additionally, their role is not one of an activity directors, though an SC does support social isolation prevention through program participation.

· Maintains updated and accurate individual files on all participating residents including individual and ADL assessments, releases, and documentation of services and community involvement.

· Maintains documentation of building programming, resident involvement, community engagement and Service Coordinator training in an accurate and timely manner according to recommended guidelines.

Requirements

  

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES):

· A bachelor’s degree in social work, Gerontology, or other related field of human services is required.

· Prior experience coordinating supportive services for seniors and/or persons who are disabled needed. 

· Strong outreach skills and experience with assessment preferred.

· Excellent oral and written communication skills needed. 

· (An equivalent combination of education & experience may also be considered). 

COMPUTER/TECHNICAL SKILLS:

· Working knowledge of Microsoft suite

· Highly analytical, technologically savvy, with strong problem-solving skills 

· Basic personal computer skills that include the ability to send/receive e-mail messages, post information to a database or spreadsheet, and perform basic word processing, bookkeeping, and/or data entry.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands or fingers, handle or feel objects, tools or controls. The employee is often required to stand, walk, sit and reach with hands. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distant vision and the ability to adjust focus. The noise level in the work environment is usually low to moderate. 

LANGUAGE SKILLS:

Ability to read, and comprehend simple instructions, short correspondence, and memos. Ability to write out routine reports, and correspondence. Ability to effectively present information in one-on-one, and small group situations to tenants, vendors, and other employees of the organization.
