Position Overview
We are seeking a compassionate, tech-savvy, and highly communicative Medicare Patient Advocate to support our value-based care initiatives. This role is focused on educating and engaging Medicare patients in programs designed to improve their health outcomes, including Advanced Primary Care Management (APCM), Remote Patient Monitoring (RPM), and voluntary alignment programs.
This individual will serve as a trusted guide for our patients—helping them understand available services, navigate care options, and feel confident participating in programs that enhance their health and well-being.
Key Responsibilities
- Proactively educate Medicare patients on care programs such as APCM, RPM, and voluntary alignment
- Clearly and confidently explain the benefits, expectations, and logistics of participation in a patient-friendly manner
- Build trusting relationships with patients, particularly older adults, through empathy and strong communication
- Assist patients with enrollment and onboarding into care programs
- Provide ongoing engagement and support, ensuring patients remain active participants in their care
- Collaborate with clinical and administrative teams to identify eligible patients and optimize outreach efforts
- Utilize technology platforms and EMR systems to track patient participation and engagement
- Address patient questions, concerns, and barriers with patience and problem-solving skills
- Reinforce adherence to care plans, monitoring programs, and follow-up recommendations
Qualifications
- Experience in healthcare, care coordination, patient outreach, or a related role preferred
- Strong understanding of (or willingness to learn) Medicare programs and value-based care initiatives
- Comfortable using technology, including EMRs, patient engagement tools, and remote monitoring platforms
- Excellent communication skills, with the ability to translate medical concepts into clear, relatable language
- Familiarity with medical terminology and clinical workflows strongly preferred
- Demonstrated ability to work with older adult populations with patience and compassion
- Confident, personable, and able to encourage participation without being pushy
- Highly organized with strong follow-through
