Responsibilities:

- Evaluate resident needs and develop personalized care plans in collaboration with our care planning team.
- Address resident, family, and physician concerns through prompt and professional communication.
- Closely monitor residents' health status, identifying potential issues and coordinating interventions accordingly.
- Maintain accurate and confidential records, adhering to strict guidelines and protocols.
- Document assessments, interventions, and follow-ups to inform care decisions and improve outcomes.
- Provide coaching and supervision to nursing support staff, ensuring they possess the necessary skills and knowledge to deliver exceptional care.
Requirements:
- A valid Registered Nurse (RN) license in the State of Colorado.
- 1-2 years of experience in an assisted living or skilled nursing setting.
- Advanced education or certification in relevant areas, such as Eden Associates, Alzheimer's and dementia certifications, or wound care programs.
- Prior experience in a supervisory capacity and proficiency in electronic medical record platforms.
- Excellent customer service, communication, computer, and database skills.
