Position Summary:
To assure completion of the State/Federal required MDS within the appropriate time frames to organize and conduct the Resident Care Conferences. To assist the individual nursing units in the use of the MDS through group and individual teaching and training. To assure the MDS is complete and accurate.
Position Responsibilities:
EXAMPLES OF WORK:
*Chairs the Resident Care Conferences that are required after each MDS is completed. Resident Care Conference is an interdisciplinary team meeting which meets daily and formally on a weekly basis to include family and resident.
*Assures completion of the MDS according to required time frames by regulation for all new admissions, upon changes in resident conditions, quarterly and annually on all residents.
*Develops an interdisciplinary care plan which is resident-oriented and individualized.
*Interacts with residents and/or their families during the care conferences and follows up on resident/family concerns.
*Provides additional supervision of the nursing units while monitoring changes in residents’ conditions.
*Meets the resident and reviews MDS prior to RCC.
*Reviews care plans for accuracy and up-dating information as residents’ conditions change.
*Reviews residents’ records and 24-hour Report Sheets for significant changes in residents.
*Demonstrates ability to “work” the RAPS and apply it to the care plan.
*Completes/assures completion of the various assessments needed with each quarterly, annual or significant change MDS assessment.
*Completes admission assessments as directed by DON and as needed.
*Provides education to nursing staff regarding completion of accurate supporting documentation as needed.
*Work closely with the ADON in connections with resident problems and changes.
*Works to achieve highest RUG category that reflects resident condition to maximize reimbursement for care provided.
*Promptly completes MDS or other assessments 100% of the time.
*Initiates assessments as appropriate based on residents’ condition and to maximize reimbursement.
*Maintain satisfactory attendance, which means six points or less in the attendance and late/leave early tracking system
*Have ability to manage stress tolerance, pressure and anxiety

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Individual must have a current Pennsylvania RN license, knowledge of State/Federal regulations, minimum of one year nursing home experience as staff/charge nurse and knowledge of the MDS system.

Salary : $37 - $39
