RN - Weekends only (Fri/Sat/Sun OR Sat/Sun/Mon) - 10 hr. shifts
Home health and hospice care
Job Responsibilities
- Work one-on-one with patients in their homes
- Covering incoming calls and making necessary patient visits as necessary
- Providing patient care and coordinating the services of a multidisciplinary team incollaboration with the patient's physician
- Completing initial and on-going comprehensive assessments of the patient's needs and requests at appropriate time points assigned
- Initiating the plan of care and revising as necessary
- Wound care, IV therapy, medication & pain management, hospice care
Qualifications and Skills
- Compassionate and empathetic
- A graduate of an accredited school of nursing and current RN license
- Prior home health, hospice, or acute care nursing experience preferred
- Computer skills
- Strong clinical and assessment ability
- Compassionate and patient-focused, positive attitude
- Reliable transportation and able to travel
- Strong documentation skills
