What are the responsibilities and job description for the RN Clinical Manager – Home Health & Hospice position at Choice Rehab LLC?
The RN Clinical Manager oversees nursing care across home health and hospice services, supporting clinicians as they deliver safe, compassionate, patient-centered care. This role supervises field staff, coordinates care with the interdisciplinary team, reviews clinical documentation, and helps ensure each patient’s plan of care reflects their needs and goals.
Minimum Qualifications:
- Supervise, coach, and support home health and hospice nurses and other assigned clinical staff.
- Review patient assessments, plans of care, visit schedules, and clinical documentation for completeness and timely follow-up.
- Coordinate with physicians, therapists, social workers, aides, and other team members to address changes in patient condition and care needs.
- Support home health admission, resumption of care, recertification, and discharge processes.
- Support hospice admissions, symptom management, care conferences, and end-of-life care coordination.
- Participate in interdisciplinary meetings and help resolve clinical or service concerns.
- Monitor quality of care, identify opportunities for improvement, and assist with staff education and performance development.
- Help ensure clinical practices and documentation align with agency policies and applicable requirements.
- Provide clinical guidance to field staff and assist with patient care when needed.
Qualifications
- Current, unrestricted Registered Nurse license in the state of practice.
- At least two years of nursing experience in home health, hospice, or a related setting; experience in both home health and hospice preferred.
- Prior clinical supervision or management experience preferred.
- Strong clinical judgment, communication, organization, and problem-solving skills.
- Ability to manage competing priorities and support staff across both service lines.
- Experience with HomeCare HomeBase or a similar electronic medical record preferred.
- Current driver’s license, auto insurance, and ability to travel within the service area as required.

Pay-Day
The Company operates a bi-weekly payroll process, paid on Tuesdays (10 days in arrears), normally 26 times annually. If one of the paydays falls on a Tuesday, Company Holiday, the

Working Hours
To meet the needs of our patients and clients, employee work hours may vary and will be set by each employee’s supervisor, manager, and operating license documentation.
Employees should consult with their supervisors regarding their schedules and time recording procedures. Choice Health at Home, the Company, is open Monday through Friday, office hours
are 8:00 am to 5:00 pm (CST). In a sales role, schedules may fluctuate.
