- Provide face-to-face care in patient homes and facilities
- Make real-time clinical assessments that guide hospice eligibility and palliative decisions
- Work directly with hospice physicians and attending providers to shape care plans
- Educate families during some of the most important conversations of their lives
- Support and elevate the interdisciplinary team with your clinical expertise
- Document and communicate findings that directly impact patient comfort and care decisions

Requirements
Current STATE RN and Advanced Practice Nurse License
Valid Driver’s License
Valid Auto Insurance
AHA CPR Certification
HomeCare HomeBase experience preferred
