The Hospice Nurse Practitioner (PRN) plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses. This position focuses on managing symptoms, providing pain relief, and supporting the physical, emotional, and spiritual needs of patients and their families during end-of-life care. The Nurse Practitioner will collaborate closely with interdisciplinary teams to develop and implement individualized care plans that honor patient wishes and improve quality of life. This role requires flexibility and responsiveness to patient needs on an as-needed basis, ensuring continuity of care in a variety of settings including home, hospice facilities, and hospitals. Ultimately, the Hospice Nurse Practitioner serves as a trusted healthcare provider and advocate, helping patients and families navigate complex healthcare decisions with dignity and respect.
As a Hospice Nurse Practitioner, you will:
Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
Perform Face-to-Face visits at the patient's place of residence.
Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
Knowledgeable of hospice criteria.

Current STATE RN and Advanced Practice Nurse License
Valid Driver’s License
Valid Auto Insurance
AHA CPR Certification
HomeCare HomeBase experience preferred
