Home Health Physical Therapist

The Home Health Physical Therapist plays a critical role in delivering personalized rehabilitative care to patients within their own homes, promoting recovery, mobility, and independence. This position requires assessing patients' physical conditions, developing tailored treatment plans, and implementing therapeutic interventions to improve strength, balance, and functional abilities. The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure comprehensive care and optimal outcomes. By providing education and support, the therapist empowers patients to manage their conditions effectively and prevent further injury. Ultimately, this role contributes significantly to enhancing patients' quality of life while reducing hospital readmissions and facilitating community reintegration.

As a Physical Therapist, you will:

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. 

• Applies concepts of infection control/universal precautions in performing patient care

• Assure continuity of quality patient care delivered with appropriate documentation

• Provide effective communication to patient/family, team members and other health care professionals

• Promote agency philosophy and administrative policies

• Maintain and complete all documentation within company stated guidelines. 

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.

• Valid Physical Therapy license

• 1 year of home health experience preferred

• CPR certification

• Valid Driver’s license

• Valid Auto Insurance

• HomeCare HomeBase experience preferred

Physical Demands

• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

• Must be able to lift, carry, pull, or push up to 100 lbs..
