We’re seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our growing team. In this role, you’ll use your clinical expertise and assessment skills to provide one-on-one care to patients in their homes, ensuring safety, comfort, and optimal recovery.
What You’ll Do
- Conduct OASIS assessments and create individualized Plans of Care tailored to each patient’s needs
- Perform skilled nursing visits, including wound care, ostomy management, IV infusions, and PICC line care
- Educate patients and families on disease management, medications, and home care safety
- Collaborate with physicians, therapists, and other team members to ensure coordinated, patient-centered care
- Document all visits, interventions, and progress accurately and on time
- Monitor patient status and promptly report changes to the appropriate provider
- Active Florida Registered Nurse (RN) license (required)
- Home Health and OASIS assessment experience (required)
- Solid clinical background with strong critical thinking skills
- Excellent communication and time management abilities
- Current CPR/First Aid certification
- Valid driver’s license and reliable transportation
