- Conduct OASIS assessments and create individualized, outcome-driven plans of care
- Evaluate patients’ physical, emotional, and environmental needs to promote independence and safety
- Provide therapy and education to help patients reach their functional goals
- Collaborate with physicians, nurses, and other members of the care team to coordinate ongoing treatment
- Monitor progress, adjust plans as needed, and maintain accurate documentation in compliance with Medicare and agency standards
- Support patients and families with compassionate, evidence-based care
- Current Florida Occupational Therapist (OT) license (required)
- OASIS assessment experience (required)
- Minimum of one year of home health or clinical experience preferred
- Strong communication, organization, and documentation skills
- Compassionate, professional, and dependable approach to patient care
