We’re seeking a compassionate, experienced Home Health Licensed Practical Nurse (LPN) to join our high-performing team. The ideal candidate has strong clinical skills, sound judgment, and outstanding communication abilities, along with a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of skilled nursing care in the home setting.
What You’ll Do
- Provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN)
- Perform nursing procedures including wound care, medication administration, injections, catheter care, and other ordered treatments
- Monitor patient conditions and report changes to the supervising RN and physician
- Reinforce patient education related to disease processes, medications, safety, and infection prevention
- Assist in implementing individualized plans of care developed by the RN
- Accurately document all patient interactions, assessments, and interventions in the EMR
- Communicate effectively with physicians, RNs, therapists, and other members of the interdisciplinary care team
- Ensure compliance with all Medicare, CMS, and home health regulations
- Current Florida Licensed Practical Nurse (LPN) license (required)
- Previous home health experience is strongly preferred but not required
- Strong clinical assessment and critical thinking skills
- Excellent communication, time management, and documentation skills
