Job Description
REPORTING RELATIONSHIP:
Supervised by: Medical Director
Positions Supervised: RN's, LPN's, Hospice Aides, Chaplains, MSW's
Interrelationships: Patients, family, IDG and other health care team members
JOB SUMMARY:
We are seeking an experienced Director of Nursing (DON) to lead out clinical operations. Choice Hospice is Joint Commission certified, having experience with previous Joint Commission survey's is preferred. This role is focused on ensuring that the patients are receiving the best care possible as well as providing leadership and guidance to the hospice team.
QUALIFICATIONS:
- Educational/Degree: Bachelor's degree in Nursing (BSN) required; MSN or equivalent preferred
- Training/Licensure: Currently licensed as a registered nurse to practice in the state.
- Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes.
- Strong knowledge of hospice regulations, standards, and best practices
- Leadership Skills with the ability to motivate and support your care team
- Experience: Minimum 5 years of experience in hospice or palliative, with at least 2 years in a leadership or management role
- Active patient contact within past three years, preferred.
JOB FACTORS:
Physical Requirements:
Requires considerable physical as needed including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100 pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations.
Mental Requirements:
Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Working Conditions:
Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.
Transportation:
Must have a current valid driver’s license, auto liability insurance and reliable transportation.
Background Screening:
All employees of Choice Hospice must have a background check completed through Michigan LARA system including fingerprinting.
Essential Functions:
- Clinical Leadership - Overseeing day to day clinical operations
- Staff Management - Recruit, train, mentor, and supervise the care team including nurses, social workers, aides, chaplains and volunteers.
- Care Planning - Review and evaluate patients Plan of Cares in coordination with IDG members
- Regulatory Compliance - Ensure and monitor compliance on state and federal level. Ensure accurate and timely documentation.
- Quality Improvement - Design and implement QAPI programs and monitor outcomes.
- Resource Management and Budgeting - Manage Staff, scheduling and assist with managing financial resources to ensure efficiency.
- Advocacy - Assist the patient and family to advocate for their unique needs both with their care with Choice Hospice and in the community
- Community Hospice Champion - strengthen relationships with community providers, facilities and organizations
- Performs emergent nursing visits as needed.
- Participates in on-call system and is responsible for providing on-call coverage when need arises.
- Other duties as assigned by Director.

Pay: $100,000.00 - $115,000.00 per year
