What You’ll Do
As a Home Health RN, you will:
- Perform comprehensive patient assessments (physical, psychosocial, environmental)
- Develop and update individualized plans of care
- Provide skilled nursing services (wound care, disease management, medication education, etc.)
- Educate patients and families on self-care, safety, and chronic disease management
- Coordinate care with physicians and interdisciplinary team members
- Supervise Home Health Aides and LPN/LVNs as needed
- Document care accurately and support payor compliance requirements
️ What We’re Looking For
- Active Texas RN license
- Ability to consistently complete minimum 15 visits per week (PRN requirement)
- Strong independent clinical judgment
- Comfortable working in home environments
- Home Health or acute care experience preferred
- Reliable transportation
Requirements
- Current RN License
- Valid Driver’s License
- CPR Certification
- Valid Auto Insurance
- At least 2 years of Home Health RN experience preferred
