We are seeking a skilled and compassionate Nurse Practitioner to join our dynamic clinical team. In this role, you will conduct Annual Wellness Visits in line with CMS standards, ensuring accurate documentation of chronic conditions for HCC risk adjustment. This role includes performing medication reconciliation, educating patients, collaborating with primary care teams to support continuity of care, maintaining precise EMR documentation, and participating in quality improvement and compliance initiatives.
Key Responsibilities
Perform physical exams, patient observations, and health assessments.
Record and update medical histories, symptoms, and treatment progress.
Create, implement, and evaluate patient care plans.
Order, administer, and interpret diagnostic tests.
Diagnose, treat, and manage acute and chronic health conditions.
Prescribe medications and coordinate follow-up care.
Monitor patient outcomes and adjust care plans as needed.
Collaborate with physicians, specialists, and interdisciplinary teams.
Educate patients and families on wellness, disease prevention, and treatment options.
Maintain compliance with HIPAA, OSHA, and infection control standards.
Qualifications
Active California Registered Nurse License (unrestricted, current, and maintained throughout employment).
Current California Nurse Practitioner License and Furnishing License.
AANP Certification required.
DEA Registration (current and maintained).
American Heart Association Certifications: BLS and ACLS required; PALS as applicable.
Minimum one (1) year of recent experience in a comparable healthcare setting.
Valid driver’s license and proof of auto insurance if travel between locations is required.
Knowledge of HIPAA, infection control, and clinical best practices.
Strong communication, problem-solving, and decision-making skills.
Proficiency in Microsoft Office and EMR systems.
