Physical Therapist – Home Health
Make a meaningful impact where patients feel most comfortable — at home. We’re looking for a compassionate and motivated Physical Therapist to join our growing home health team and help patients regain mobility, restore independence, and improve their overall quality of life.
What You’ll Do
As a Physical Therapist, you will provide individualized rehabilitative care to patients recovering from illness, injury, or surgery in the comfort of their homes. Responsibilities include:
- Perform and document comprehensive initial evaluations and assessments
- Develop and implement personalized treatment plans to improve mobility, strength, balance, and function
- Monitor patient progress and modify treatment plans to maximize outcomes
- Educate patients and families on exercises, safety, and ongoing care needs
- Maintain accurate and timely clinical documentation
- Collaborate with physicians, caregivers, and interdisciplinary healthcare teams
- Follow infection control and safety protocols at all times
- Support the agency’s mission of delivering compassionate, high-quality patient care
Qualifications
- Graduate of an accredited Physical Therapy program
- Current state Physical Therapist license
- CPR Certification required
- Valid driver’s license and auto insurance
- Home health experience preferred
- HomeCare HomeBase experience preferred
Physical Requirements
- Ability to stand, walk, bend, kneel, squat, and climb for extended periods
- Ability to lift, carry, push, or pull up to 100 lbs.
