What You'll Do
As a Home Health Physical Therapist, you will:
- Perform comprehensive initial evaluations to assess patient needs and develop individualized treatment plans.
- Design and implement evidence-based therapy programs to improve mobility, strength, balance, function, and overall quality of life.
- Monitor patient progress, adjust treatment plans, and ensure optimal outcomes.
- Educate patients and family members on exercises, safety techniques, and home programs.
- Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care.
- Maintain accurate, timely clinical documentation in accordance with company policies and regulatory requirements.
- Follow infection control and patient safety standards during every visit.
- Deliver compassionate, patient-centered care while promoting our mission and values.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current, unrestricted Physical Therapist license in the state of practice.
- CPR Certification.
- Valid driver's license and reliable transportation with current auto insurance.
- One (1) year of Home Health experience preferred.
- Homecare Homebase experience preferred.
Physical Requirements
- Ability to stand, walk, sit, bend, kneel, squat, crawl, and climb throughout the workday.
- Ability to safely lift, carry, push, and pull up to 100 pounds with or without reasonable accommodation.
