About this Clinical Sales Role:
The LVN Marketing Liaison plays a critical role in bridging the gap between patients, healthcare providers, and home health services to ensure seamless and effective patient care. This position involves conducting comprehensive patient assessments, coordinating care plans, and facilitating communication among multidisciplinary teams to optimize health outcomes. The LVN Liaison serves as a primary point of contact for patients and their families, providing education and support throughout the care continuum. By leveraging clinical expertise and strong interpersonal skills, the LVN Clinical Liaison helps identify patient needs, advocates for appropriate services, and promotes adherence to treatment protocols. Ultimately, this role contributes to enhancing patient satisfaction and improving overall quality of care within the home health setting in the UT SW Hospital System. *Not under the employment of UTSW
Qualifications:
- Current and valid Licensed Vocational Nurse (LVN) license in the state of Texas.
- Minimum of 2 years of clinical nursing experience, preferably in home health, hospice, or related healthcare settings.
- Basic knowledge of home health regulations, patient care standards, and clinical documentation requirements.
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
- Ability to travel within the assigned geographic area to conduct patient visits and meetings.
Preferred Qualifications:
- Experience working as a Clinical Liaison or in a similar care coordination role within home health services.
- Certification in case management or care coordination (e.g., CCM, CMC) is highly desirable.
- Familiarity with electronic medical records (EMR) systems and proficiency in documentation software.
- Bilingual skills, particularly in Spanish, to better serve diverse patient populations.
- Demonstrated ability to manage multiple priorities and work independently in a fast-paced environment.
Responsibilities:
- Conduct thorough patient assessments in home and clinical settings to evaluate health status and care needs.
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to develop and implement individualized care plans.
