Schedule: Mon-Fri 8 am - 5 pm

A Brief Overview Serves as a contributing member on the Behavioral Health team; provides screening and assists with patient disposition plans that present with a health crisis. Helps Behavioral Health patients connect with resources that allow them to manage mental crisis as they arise; improve overall health and potentially avoid hospital encounters. Assists with posting discharge care and connecting patients to community resources and supports for individuals with complex circumstances. Essential Functions
- Provides brief interventions and psychoeducation to support patient wellness and recovery.
- Works in a highly collaboratively manner with therapy clients' assigned case managers, and other service providers, within and outside facility.
- Assists in developing measurable individualized therapy goals and objectives.
- Serves as the primary contact for all payors regarding utilization review and management issues.
- Delivers support of clinical integration needs of other clinical services, and collaboration with medical director leadership.
- Maintains a patient first philosophy and engages in service recovery when necessary.
- Participates in clinical huddles when working with medical & behavioral health providers to review the appointment schedule for the day and determine which patients will need visits.
- Ensures that documentation compliance requirements are met consistently.
- Bachelor's Degree from an accredited college or university in Social Work or related field Required
- Experience working in a clinic or hospital setting Preferred
